Job Description:

The Registered Nurse will provide intake, assessment, and care management services to clients, which includes ongoing assessment for program eligibility, monitoring all aspects of care and client services (social and medical), and ensuring needs of clients and caregivers are met through a collaborative process.
REQUIREMENTS:

-OH licensed Registered Nurse in good standing
-At least one year of experience working with persons with disabilities/chronic conditions and LTSS
-Prior case management experience and electronic documentation preferred
-Valid driver s license with good record and reliable transportation
